Vendor Tasting Agreement*

Samples against Stigma benefit will be held on Wed., May 20, 2020 from 5pm - 8pm at the Executive Court Banquet Facility in Manchester. By completing this form, you are agreeing to participate in this MHCGM fundraiser, which will help to provide mental health services for children and teens in the greater Manchester communities. Your partnership would require you to prepare quantity servings for approximately 200 - 250 attendees and abide by Manchester Health Dept. regulations. Please write the name of your business in the field below.

Serving Options*

A representative will be completely set up at their assigned station no later than 4:30 pm and will be present through the entire event to distribute samplings. *(Preferred option)
We will prepare samplings for a 3:45 pm pick-up and consent for a volunteer to distribute our product during the event.
