- Medical clearance from primary care doctor to participate
- Diagnosed and living with mental health challenges

The Endowment for Health of New Hampshire gathered Monadnock United Way, the Monadnock Community Foundation, the NH Charitable Foundation, and the Hoffman Family Foundation into a funding coalition that became InSHAPE’s Local Initiative Funding Partner with the Robert Wood Johnson Foundation.

Dartmouth’s Center for Health and Aging has been studying the effectiveness of InSHAPE, under the leadership of Stephen J. Bartels, MD, MS and Sarah I. Pratt, PhD, since the original 2003 pilot study and in subsequent research funded by CDC, NIH and CMS.
