A new Intensive Transition Team has been added as a service of The Mental Health Center, which will support high-risk children and adults who are discharged from local emergency rooms. The new team will serve as a “safety net” for certain target populations and will help to improve transitions of care from acute services to primary care and outpatient behavioral health settings.
