LifeConnections provides a wide range of specialized services to people of all ages and abilities in NH and is best known for creating individualized service options for people with complex behavioral health and/or medical needs who may be at risk for moving to higher levels of care. In partnership with NH’s Medicaid MCO’s, LifeConnections launched the CARE program, an intensive in-home service for youth. The CARES program provides short-term 14-day intensive home-based interventions to proactively reduce crisis situations, prevent emergency department visits and shorten inpatient hospitalization stays for youth in crisis and in need of supports to return home safely. Youth and families will receive daily wraparound support services provided by a multi-disciplinary team. CARES will support youth discharged from Emergency Department within 24 hours of the referral to successfully support them in the family home. The CARES program will be presented by Dr. Thomas Vincent, Psy. D Chief Clinical Officer, Katherine Jordan, Associate Executive Director of Youth Services and Jennifer Cordaro, M.Ed Executive Director of LifeConnections.
