Provides a full range of community-based psychiatric services for adults with severe and persistent mental illness. The goal is to promote recovery by identifying and reducing the major symptoms of mental illness while restoring and improving life skills for the individual. Along with traditional case management services, clinical case managers, utilizing evidence-based practices whenever possible, provide therapy and work with clients to coordinate medications, housing, employment, finances, and leisure time/social activities. All prospective consumers are also screened for substance use/abuse problems and an integrated treatment plan is developed when appropriate.

 

Intensive case management teams are available to serve “hard-to-reach” individuals who are difficult to engage due to: a reluctance to accept treatment; severe substance abuse; homelessness; or co-occurring major physical problems. A team offering in-home supports for persons requiring assistance with treatment and/or activities of daily living is also available. These specialized services are provided seven days a week.

 

The purpose of Community Support Services is to employ a “Best-Practice” approach to enabling persons affected by severe and persistent mental illness to live and work independently in the community and achieve an optimum level of wellness.
